Inspection at your Office Location. Call the local Fire
Division at your town to schedule and appointment for
inspection. You must have a Fire Extinguisher located
at the office by the time of inspection. After the inspection
you should receive a Permit Certificate from the Fire
Rescue Department

You must submit Fingerprint card with the application
for screening purposes.

your Oxygen Application. The cost of the application

of Florida Department of Health Drugs, Devices and Cosmetics.
You will receive a Permit in a Period not greater than
a month.

After this we will be submitting the AHCA application.
The cost of this application is $339.00.
